• ALL FORMS COME DIRECTLY TO NOHO RATHER THAN THE CASTING DIRECTOR/YOU – WE’LL THEN ADD TO YOUR FINAL LINK THAT WE SHARE FROM THE START
• NoHo Casting will upload completed forms with headshot if requested to your secure link

The IPA artist declaration form is available via a fillable PDF form (N.B. we can also create a filable PDF form for the headsheet, combining both into one PDF)
